What is a fitness-for-work certificate for mast work?

A fitness-for-work certificate (tjänstbarhetsintyg) shows that you have completed the medical check required for work that involves climbing with a large height difference, and that a doctor has assessed your fitness for that work.

In everyday language, people often use terms such as:

  • medical certificate for mast work
  • climbing with a large height difference
  • mast certificate
  • work at height certificate
  • mast and pole work
  • medical certificate for work at height

The formal term in the Swedish Work Environment Authority’s regulations is fitness-for-work certificate for climbing with a large height difference.

When is a medical certificate required for work at height?

The requirement applies to work that involves climbing with a height difference of more than 13 metres, for example on:

  • masts
  • poles
  • ladders
  • trees
  • or similar work environments

There must also be a risk that the worker could fall freely or uncontrollably, or end up in a situation where rescue is difficult.

This means that not all work at height automatically requires a fitness-for-work certificate. The type of climbing, the height difference and the risks involved decide whether it applies.

Who needs a fitness-for-work certificate for mast work?

The medical check may be relevant for people who work with:

  • telecommunications masts
  • radio and mobile masts
  • power line poles
  • electrical and energy installations
  • wind power installations
  • work on tall poles
  • tree work and advanced tree climbing
  • other work that involves climbing with a large height difference

The employer is responsible for identifying which employees the requirement applies to and for making sure the medical check is carried out.

How the medical check works

The purpose of the examination is to find out whether you have any medical condition that could increase the risk during physically demanding work and climbing at height.

Health declaration and medical review

The doctor goes through, among other things:

  • previous and current working conditions
  • previous and current illnesses
  • medicines
  • tobacco use
  • alcohol and drug habits

The doctor pays particular attention to conditions that could cause, for example, sudden loss of consciousness, a drop in blood pressure or other sudden weakness.

Heart and lung examination

The doctor examines your heart and lungs, among other things.

Work on masts and at height is often physically demanding. That is why it matters to identify medical conditions that could increase the risk while you work.

Blood pressure and orthostatic test

The doctor checks your blood pressure and assesses whether you are prone to orthostatic reactions.

This means checking whether your blood pressure changes in a way that could make you dizzy or faint when you change position.

ECG and blood lipids

The first medical check, before you start the work, includes a resting ECG and a blood lipid test.

At later checks, resting ECG and blood lipids are tested:

  • at least every five years if you are under 40
  • at least every two years from the age of 40

Cardiovascular risk assessment

The examination also includes an age-adjusted assessment of your risk of cardiovascular disease.

If you are 40 or older, the assessment must include an established risk assessment tool.

If the examination shows an increased cardiovascular risk, you may need further tests before the doctor can decide on your fitness for work.

The fitness-for-work certificate is issued

When the examination is complete, the doctor makes an overall medical assessment.

If your health is compatible with the work, the doctor can issue a fitness-for-work certificate.

The certificate shows that the medical check has been carried out and the latest date for your next check.

How long is the fitness-for-work certificate valid?

The medical check must be carried out:

  • before you start the work in question
  • and then at least every 12 months

This means your fitness for work normally needs to be assessed every year.

The doctor can decide that your next medical check must take place sooner than 12 months.

Frequently asked questions about medical examinations for work at height

How high can you work without a fitness-for-work certificate?

The specific requirement for a medical check for climbing with a large height difference applies at a height difference of **more than 13 metres**, when there is also a risk of a free or uncontrolled fall, or when rescue could be difficult.

Other work environment rules may of course apply to work at lower heights too.

Do I need a medical certificate to work on masts?

If the work involves climbing with a height difference of more than 13 metres and meets the other criteria in the Swedish Work Environment Authority’s regulations, you need a medical check and a fitness-for-work certificate.

How often do I need a health check for mast work?

The check must be done before you start the work and then at least every **12 months**. The doctor can decide on a shorter interval.

Is an ECG included in the medical examination?

Yes. A resting ECG is included in the first medical check. After that, the ECG is repeated at age-based intervals.

If you are under 40, the interval is at most five years. From the age of 40, it is at most two years.

Is an exercise ECG or fitness test included?

An exercise ECG or fitness test is not a mandatory standard part of the fitness-for-work assessment for climbing with a large height difference under AFS 2023:15.

However, further tests may be needed if the doctor finds an increased medical risk.

Is spirometry included?

Spirometry is not listed as a mandatory standard part of the current medical check for climbing with a large height difference.

The doctor carries out a clinical examination of your heart and lungs and can add further tests if needed.

What is the difference between a mast certificate and a fitness-for-work certificate?

“Mast certificate” and “medical certificate for mast work” are common names. The formal certificate under the Swedish Work Environment Authority’s regulations is a fitness-for-work certificate for work that involves climbing with a large height difference.

Can employers book examinations for several employees?

Yes. Companies that need medical checks and fitness-for-work certificates for several employees can contact Läkarintyg.se to make a company booking.
