Before Your Visit

Bring valid ID.
Bring a list of medications you take regularly.
Bring any health form the IVF clinic requires to be filled out.
If you're unsure about the contents of the IVF clinic's request, you should always email the form to us in advance.
We arrange referrals for lab testing and any required X-rays (drop-in) to a separate facility of your choice. There are many such facilities, and in some cases your own primary care clinic may be the most convenient option.
If your case is urgent, the lab testing/X-ray referral can be sent before your appointment, and you can have the tests done the same day.
